What Mounjaro is

Mounjaro contains tirzepatide, a long-acting medicine that acts on two hormone receptors, GIP and GLP-1. In the UK, where this KwikPen is authorised, it is used:

  • Type 2 diabetes: in adults and in children aged 10 and over whose diabetes is not well controlled, alongside diet and exercise, either on its own when metformin is not suitable or together with other diabetes medicines.
  • Weight management: in adults with a body mass index (BMI) of 30 kg/m² or more, or 27 to under 30 kg/m² with at least one weight-related health problem, alongside a reduced-calorie diet and more physical activity.

Whether Mounjaro is right for you is a decision for your prescriber.

Product details

  • Active ingredient: tirzepatide.
  • Strengths: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg or 15 mg in each 0.6 ml dose.
  • Pack: 1 KwikPen containing 4 once-weekly doses. Any medicine left in the pen after the fourth dose must be thrown away.
  • Other ingredient to know about: each dose contains 5.4 mg of benzyl alcohol, which can cause allergic reactions.
  • Manufacturer: Eli Lilly.

Dose and dose steps

Only use the strength your prescriber has written. The usual dose plan in the UK product information is:

  • Start with 2.5 mg once a week for 4 weeks. This is a starting dose, not a maintenance dose.
  • Then increase to 5 mg once a week.
  • If needed, the dose can be increased in 2.5 mg steps (7.5 mg, 10 mg, 12.5 mg, 15 mg), with at least 4 weeks on each dose before the next increase.
  • For adults, the maintenance doses are 5 mg, 10 mg or 15 mg once a week. The maximum dose is 15 mg once a week.
  • For children and teenagers aged 10 to 17 with type 2 diabetes, the maintenance dose is 5 mg or 10 mg, and the maximum is 10 mg.
  • For weight management, your prescriber will review whether to continue if you have not lost at least 5% of your starting weight 6 months after reaching your highest tolerated dose.

How to use the KwikPen

  • Inject once a week, on the same day each week.
  • Inject under the skin of your stomach area (abdomen) or thigh. Someone else can inject into the back of your upper arm. Change the injection site each week.
  • Read the Instructions for Use that come with the pen carefully before your first injection.
  • Missed a dose? Take it as soon as possible within 4 days of the missed day. If more than 4 days have passed, skip it and take your next dose on your usual day.
  • Changing your injection day: you can, as long as there are at least 3 days between two doses.
  • Do not use the pen if it is damaged or the liquid is cloudy, discoloured or contains particles.

Storage

  • Keep in a refrigerator at 2 °C to 8 °C. Do not freeze; do not use a pen that has been frozen.
  • After first use, the pen can be kept out of the fridge at up to 30 °C for up to 30 days, then must be thrown away.
  • Keep out of the sight and reach of children and do not use after the expiry date.

Possible side effects

Very common (may affect more than 1 in 10 people): nausea, diarrhoea, vomiting, constipation, stomach pain, decreased appetite, tiredness, and low blood sugar when used with a sulphonylurea or insulin.

Common (may affect up to 1 in 10 people): allergic reactions, dizziness, low blood pressure, indigestion, bloating, burping, wind, acid reflux, gallstones and gallbladder inflammation, hair loss, injection-site reactions, faster heart rate, raised pancreatic enzymes, and low blood sugar when used with metformin or an SGLT2 inhibitor.

Get urgent medical help if you have severe, persistent pain in the stomach area, with or without nausea and vomiting (possible pancreatitis: stop using Mounjaro), or signs of a severe allergic reaction such as difficulty breathing, rapid swelling of the lips, tongue or throat, difficulty swallowing or a fast heartbeat.

Nausea, vomiting and diarrhoea can make you lose fluid, which can affect your kidneys. Drink plenty of fluids, especially when you start treatment.

Before you use Mounjaro

  • Do not use it if you are allergic to tirzepatide or any of the other ingredients.
  • Tell your prescriber if you have had pancreatitis, have diabetic eye disease (retinopathy), have kidney problems, or take insulin or a sulphonylurea (your other medicine dose may need lowering to avoid low blood sugar).
  • Tell your doctor that you use Mounjaro if you are going to have an operation under general anaesthesia or deep sedation.
  • Pregnancy: Mounjaro must not be used during pregnancy and is not recommended if you could become pregnant and are not using contraception. Stop it at least 1 month before a planned pregnancy.
  • The contraceptive pill: Mounjaro can reduce how well the pill is absorbed. If you are overweight or have obesity and use the pill, a non-oral method or an added barrier method is advised for 4 weeks after starting and for 4 weeks after each dose increase.
  • Breastfeeding: talk to your doctor first.

Frequently asked questions

Do I need a prescription to buy Mounjaro in Kenya?

Yes. Mounjaro is a prescription-only medicine. We only supply it after our pharmacist has checked a valid prescription from a licensed prescriber.

How long does one Mounjaro pen last?

Each KwikPen holds 4 once-weekly doses, so one pen lasts about 4 weeks at one strength.

Which strength do I start on?

Treatment starts at 2.5 mg once a week for 4 weeks, then moves to 5 mg. Always buy the strength your prescriber has written.

How often is the dose increased?

Not more often than every 4 weeks, in 2.5 mg steps, and only if your prescriber decides to increase it.

What is the highest dose?

15 mg once a week for adults. For children aged 10 to 17 with type 2 diabetes the highest dose is 10 mg.

What if I miss a dose?

Take it within 4 days of the missed day. If more than 4 days have passed, skip it and take the next dose on your usual day.

Can I change the day I inject?

Yes, as long as there are at least 3 days between two doses.

Where do I inject Mounjaro?

Under the skin of the stomach area or thigh, or the back of the upper arm if someone else injects. Change the site each week.

Does Mounjaro need to be kept in the fridge?

Yes, at 2 °C to 8 °C and never frozen. After first use it can be kept below 30 °C for up to 30 days.

What are the most common side effects?

Nausea, diarrhoea, vomiting, constipation, stomach pain, reduced appetite and tiredness. Drink plenty of fluids and speak to your pharmacist or doctor if they bother you.

When should I get urgent help?

For severe, persistent stomach pain, or signs of a severe allergic reaction such as swelling of the face or throat or difficulty breathing.

Can I use Mounjaro with insulin or other diabetes medicines?

Your prescriber decides. With insulin or a sulphonylurea the risk of low blood sugar is higher, so their dose may need lowering.

Does Mounjaro affect the contraceptive pill?

It can reduce how well the pill works. A non-oral or added barrier method is advised for 4 weeks after starting and after each dose increase.

Can I use Mounjaro if I am pregnant or planning a pregnancy?

No. It must not be used in pregnancy, and should be stopped at least 1 month before a planned pregnancy.

Are pen needles included?

Yes. Pen needles are supplied with every Mounjaro KwikPen order from HealthSpot.

When do I pay, and what if my prescription is not approved?

You pay only after our pharmacist approves your prescription, by M-Pesa, card or cash on delivery. If it can't be approved, you pay nothing and our pharmacist will explain why.
